Not sure if I'm supposed to do something here. I gave my referral link to my cousin who wanted to rent movies to watch since he said he's always bored. Someone signed up a couple days after I gave him the link and the name of the person wasn't his and some different name so I asked him why he used a fake name. He told me he never signed up and gave my referral link to a friend in Nepal. I have a referral sitting there not verified that is not from the U.S. Do I just let it sit there or do I contact SAD to delete him? As far as I know, only people from the U.S. can join this site correct?

This is a situation for SAD to take of.  There is no need for you to do anything (especially since it's not your account).  Once they realize it is not a US account, it will be cancelled.
